Second DUI Conviction In Another State

If the suspension is the result of a DUI conviction in another state and a
prior DUI conviction is on your driving history, the duration of suspension
is three years from the effective date of suspension. However, if the
operator is under 21 years of age, the duration of suspension is three
years from the effective date of suspension or until the date of the
operator’s 21st birthday, whichever is longer.

Before a license can be restored, a check or money order in the amount
of $125, payable to "DMV", must be mailed, along with any necessary
documents at least two weeks prior to the eligibility date to the following
address:

Note:  Please clearly print your name, current address and date of birth
on all correspondence. You cannot operate a motor vehicle until you
receive an official notice from the Department of Motor Vehicles
informing you that the restoration has been processed.
